Description of Research
Sweet and Brittle: Bone Health in Type 1 Diabetes (T1D). Men and women living with T1D have a two- to four-fold higher risk of any fracture, and up to a seven-fold higher risk of hip fracture compared to those without diabetes. We conducted a Canadian survey that revealed that most people who live with T1D are unaware and have not been informed by their doctors that diabetes increases their risk of fracture (1). We have reported that femoral neck bone mineral density, lumbar spine trabecular bone score (reflecting bone quality), and serum bone formation and bone resorption markers are all significantly lower in people with T1D vs controls of similar age, sex, and BMI who were part of the DenSiFy cohort (2). In the same cohort, we reported that Advanced Glycation End Products (AGEs) were not independently associated with bone outcomes in adults with relatively well-controlled T1D (3). We are now analysing the changes in bone outcomes over a 4-year period in the DenSiFy cohort (funded by Diabetes Canada: NCT06351176). We are also conducting a pilot study evaluating the short-term impact of improved glycemic control with automated insulin pump therapy on serum bone turnover markers in T1D (NCT06158503): stay tuned for upcoming presentations and publications of the results!

Impact of obesity, GLP-1 medicines and bariatric surgery on bone health. We showed that fracture risk in obesity is skeletal-site specific and varies according to the definition of obesity (BMI vs waist circumference) (4,5,6). Bariatric surgery, especially malabsorptive procedures, is associated with an increased risk of fractures. GLP-1 medicines may lead to muscle and bone loss, and increase fracture risk, especially if weight loss is large, in older individuals, and in those at high fracture risk (7). Upcoming results from our multicentre observational cohort of patients looking at the longer-term impact of sleeve gastrectomy on bone and muscle outcomes (funded by CIHR: NCT06547515)!
